Based on observation, interview, and record review, the licensee did not comply with the section cited above in missing sleep logs for 2 of 2 infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete sleep logs and send copy of LPA.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in missing updated immunization for C1 and C6, as well as missing PM 286 (6/95) for C6 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain updated immunizations and "blue card" and send proof to LPA.

Based on record review, the licensee did not comply with the section cited above in missing LIC627 for C1 and incomplete LIC 627 FOR C6 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LPA with proof of completed LIC 627 for both children.

Based on observation, the licensee did not comply with the section cited above in that upon LPA's arrival, licensee was in the front yard with an infant while four children ages 3-4 were asleep inside, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA consulted with licensee during inspection.

Based on record review, the licensee did not comply with the section cited above in that licensee and S1 have expired mandated reporter certificates, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of certificates to LPA by 5/31/2024.

Based on observation, the licensee did not comply with the section cited above in that the stairs do not have a gate. Children pass the stairs when entering/exiting home and there isn't another safety gate to the dining room where the stairs are located. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a photo of gated stairs to LPA by 5/31/2024.

Based on observation, the licensee did not comply with the section cited above in that three children ages 3-4 were asleep in infant devices, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit photo of cots/mats to LPA by 5/31/2024.

Based on record review, the licensee did not comply with the section cited above in that not all children (past and present) were listed on the roster, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit current roster to LPA by 5/31/2024.

Based on observation, the licensee did not comply with the section cited above in that the cupboard under kitchen sink (cleaning products and bleach) was left unlatched; knife drawer was unlatched - within reach of children. Children were asleep during inspection therefore not an immediate risk. This posed a potential health, safety or personal rights risk to persons in care.

Resolution: Items were moved during inspection.

Based on observation, the licensee did not comply with the section cited above in that bleach was left in an unlatched cupboard and insecticide was left in the backyard. Children were asleep during inspection therefore not an immediate risk. This posed a potential health, safety or personal rights risk to persons in care.

Resolution: Items were removed during inspection.
