Based on interview, and record review, the licensee did not comply with the section cited above. The provider stated that staff/herserlf are physically checking on the infant every 15 minutes when infant is asleep; however, there is no documentation demonstrating that 15 minute checks are being conducted. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: On this day, Licensee began to document the 15 minute checks on a non-legal document provided by LPA Iglesias. Licensee also provided a writtent statement, indicating that she will physcially checking on the infant every 15 minutes when infant is asleep, she is going to document it, and she is going to keep the document on the infant's file.

Based on interview and record review, the Licensee did not comply with the section cited above. The Licensee and her staff did not complete the mandated reporter training. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and her staff will complete the mandated reporter by enrolling at www.mandatedreporterca.com. Additionally, on this day Licensee wrote a statement indicating she understands that mandated reporter is to be completed every two years. Licensee will be sending proof of correction to CCL by 11/23/2021.

Based on interview and record review, the licensee did not comply with the section cited above. The Licensee did not have and Individual Infant Sleeping plan for the infant in-care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: On this day, the Licensee was provided with an Individual Infant Sleeping Plan. The Licensee stated she will be addressing the matter with the infant's parent(s) and have the parent(s) complete the Infant Sleeping Plan by or before 11/12/2021. Licensee will provide CCL with proof of correction by the date listed above.
