Based on interview and record review, Licensee did not ensure that parents were allowed to enter the facility during the COVID Pandemic. Parents are to be allowed entrance into the facility as it is their right as an authorized representative. This posed a potential risk to the health, safety and personal rights of children in care.
Based on observation and interview, the licensee did not comply with the section cited above in coast guard approved life ring blew away in wind storm and missing from pool area, which poses/posed a potential health, safety or personal rights risk to persons in care.

(a)(1) Personal Rights: The licensee shall ensure that each child is accorded the following personal rights: To be accorded dignity in his/her personal relationships with staff and other persons. This requirement was not met as evidence by Staff # 1 disclosed covering child's # 1 mouth to keep child from speaking also grabbing child by the lapel of jacket, which poses a potential risk for children in care.
Limitations on Capacity (a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. Based on observation and interview this requirement is not met as evidence by: LPA observed 6 preschool children in the Kindergarten classroom which is not licensed by our department. This posed an immediate risk to the health, safety and personal rights of children in care.
