Based on the observations , interviews , records obtained facility left child #1 unattended in the classroom. This is an immediate risk to the health ,safety , and personal rights of children in care .

: Based on the interviews staff school age children were allowed to be in the facility during the day care hours when they were sick or no afterschool program , which poses potential risk to the health , safety and personal rights of children in care .

101238.2 (d)(2) Outdoor Activity Space (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement was not met evidenced by: Multiple incdents were recorded in the area where uneven pavement and sand pit . LPA observed incident reports in the children`s files and verified with staff place of the incident. Per staff uneven pavement is the area where most of the incidents happened. Disclosures were made that it could be at least 1-3 incidents per week .

(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.

Based on observation, Licensee did not ensure that electrical outlets were covered in the classrooms. Outlets were observed with no covers on 02/07/2024, 02/23/2024, and 03/07/2023. This is a potential risk to the health, safety, and personal rights of children in care.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met evidenced by: Based on interviews and record review, one child was left outside in the play yard alone without adult supervision for 15-20 minutes, which poses an immediate risk to the health and safety of children in care.
