Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 5 children were missing an Infant individual sleep plan (LIC 9227) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure the parent of C1 completes, signs and returns LIC 9227 to facility. Licensee will send proof to LPA by POC date of 1/12/26

Based on observation, interview, record review, the licensee did not comply with the section cited above in 3 of 5 children did not have 15 minute safe sleep documented, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will start documenting infant safe sleep for infants who are newborns to 24 months and send LPA proof of documentaion by POC date of 01/16/2026

Based on observation, interview, record review, the licensee did not comply with the section cited above in 4 out 4 staff could not provide proof of completed Mandated Reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure that all staff complete mandated reporter training and send LPA proof of certificated by POC date of 01/23/2026

Based on observation, interview, record review, the licensee did not comply with the section cited above in 2 out of 4 staff could not provide proof they have been immunized against influenza (or declination), pertussis, measles and TB, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure that staff 2 and Staff 4 obtain proof they have been immunized against influenza (or declination), pertussis, measles and TB by POC date of 02/09/26.

Based on observation, interview, record review, the licensee did not comply with the section cited above in 4 out of 5 children were missing immunization record for LPA to observe, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain proof of immunization records for C1, C2, C4 and C5 and send proof to LPA by POC date of 02/09/26

Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out 5 children did not have a signed LIC 995 in their file for LPA to observe, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will ensure the parent of C1 signs and returns LIC 995 to facility. Licensee will send proof of complete LIC 995 to LPA by POC date 01/12/26

Based on observation, interview, record review, the licensee did not comply with the section cited above in a baby walker and infant swing/rocker in an accessible area, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will write a declaration stating that baby walkers and infant swings/rockers have been removed from the home and will not be use for children in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in all staff files did not have a current Mandated Reporter Child Care Provider (AB1207) training completed, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will have all staff complete the training and submit proof to LPA by POC date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 5 of 6 Child Files, Children #2-6, did not have immunization records present, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will speak with the families and obtain the immunization records for Children #2-6 and submit proof to LPA by POC date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 Staff, did not have current Pediatric CPR/First Aid Certificates on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will make sure personnel completes in person Pediatric CPR/First Aid training and will submit proof to LPA by POC date.

Based on record review, the licensee did not comply with the section cited above in 7 out of 8 Children's Files did not have immunization records present, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will have parents bring or send copies of their child's immunization records. Licensee will send copies to LPA by POC date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 Staff, Staff #1 and #2, did not have Mandated Reporter Child Care Provider Training completed in their file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, all personnel will complete the Mandated Reporter Child Care Provider (AB1207) training and submit proof to LPA by POC date.
