Based on children files review, the licensee did not comply with the section cited above and licensee did not complete infant nap chart for currently enrolled three infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, while infants are napping check on them every 15 minutes but did not complete nap chart recently. Per licensee, effective today she will complete nap chart for infants up to two years old and a copy will be submitted to LPA via email by 02/26/26.

Based on licensee's assistant file review, the licensee did not comply with the section cited above licensee's assistant Armine Uguryan did not have proof of immunization against pertussis and measles on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, her assistant, Armine Uguryan will have proof of immunization against pertussis and measles on file by 02/26/26 and copies will be emailed to LPA by 02/26/26.

Based on facility file review, the licensee did not comply with the section cited above and did not have current facility roster during this inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During this inspection licensee updated her facility roster and it was completed and current.

Based on children files review, the licensee did not comply with the section cited above in 1 out of 1 infant under 12 months old did not have required Individual Infant Sleep Plan LIC 9227 on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will have Individual Infant Sleep Plan LIc 9227 for child #1 on file by 02/26/26 and a copy will be emailed to LPA by 02/26/26.

Based on record review, the licensee did not comply with the section cited above and her required mandated reporter training certificate was completed on 12/07/2019 and was not updated every two years as it is required, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, she will update her required mandated reporter training and will submit a copy of certificate to LPA Eivazian by plan of correction due date, 04/29/2022.

Based on record review child#4 did not have immunization record on file, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, child#4's parent submit a religious exemption letter to licensee. Per licensee, she will talk to child#4's parent in order to submit a copy of immunization record by plan of correction due date or child#4 will be terminated from daycare.
