Based on observation, the licensee did not comply with the section cited above. LPA observed CLOROX bleach, GAIN detergent, Febreeze odor eliminator in an accessible closet in the hallway and hygiene products such as toothpaste, mouthwash, facial cleansing pads, and razors in the restroom. There were also knives accessible to children in care in the kitchen which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to child proof or remove the items from the restroom, add a child proof latch to the closet in the hallway, and make the knives inaccessible in the kitchen by 07/03/2025.

Based on observation, the licensee did not comply with the section cited above. LPA’s observed in dining room Charcoal lighter fluid- combat max roach killing gel and hot shot roach bait that was accessible to children in care. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA advised that poisons are to be kept locked and inaccessible to children in care. POC cleared on todays date.

Based on observation, the licensee did not comply with the section cited above. Licensee did not have a operable Carbon Monoxide detector. Licensee replaced the batteries to the equipment but the detector was not fully operational. This posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will replace nonfuctional carbon monoxide detector and will submit proof to CCL.
