Based on observation and interview, the licensee did not comply with the section cited above in that 1 out 1 infant (age 5 months) did not have a crib or play yard available for napping which posesd a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, a play yard will be obtained and available for Infant prior to the next time they are present in daycare. A photo of the play yard as proof of purchase will be sent to LPA by POC due date.
Based on interview and record review, the licensee did not comply with the section cited above in that 1 out of 4 children's files reviewed did not have immunization record which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the immunzation record for child #1 wil be obtained by POC due date 12/17/25, A subsequent visit will be conducted to review file for Child #1.
Based on interview and record review, the licensee did not comply with the section cited above. Per Licensee does not know if she has taken influenza, pertussis, and measles and has no proof which poses a potential health and safety risk to persons in care.
Resolution: Per Licensee will get blood work to see if she has ever taken influenza, pertussis, and measles. Licensee will then submit proof of immunizations to LPA via email by POC due date of 01/06/2023.
