Based on interview and record review, the licensee did not comply with the section cited above in that 2 out of 2 staff providing care have Pediatric CPR/1st Aid that has epired in July 2025, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the Pediatric CPR/1st aid will be completed by licensee and Staff 1 and proof of completion will be sent to LPA by POC due date.
Based on interview and record review, the licensee did not comply with the section cited above in that Staff 1 did not have proof of required immunizations, MMR and TDaP, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, proof of required MMR and TDap Staff 1 will be sent to LPA via email by POC due date.
Based on observation, the licensee did not comply with the section cited above in that the backyard has which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The deficiency was corrected during inspection. The licensee removed chainsaw to locked storage room and placed a safety latch on refrigerator to maintin inaccesible to children in care.
Based on observation and interview, the licensee did not comply with the section cited above in the there was an unopened container of allergy medicince a children's cubby, accessible to children in care which posed a potential health, safety or personal rights risk to persons in care.
Resolution: The deficiency was corrected during visit, the license removed the medication and placed in inaccessible cabinet in the kitchen. Per licensee, they will make sure to communicate with parents and check children's cubbies to ensure there are no hazardous items brought from home.
Based on interview and record review, the licensee did not comply with the section cited above in that 1 out of 7 children present did noth have immunization record which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, the records will be obtained and a copy will be sent to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in due to the outdoor play space had the following: Rotting fruit from tree on the grass, a wheel barrow placed upside down, air conditiing unit not covered, sprinkler with wires sticking out. Clutter on top of bar area should be removed to aviod it falling on children, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, the outdoor play area will cleaned, the AC unit covered, wires will be covered, item on top of bar will be removed and photos as proof of correction will be sent to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in that the barrier (play yard) blocking stairs leading to 2nd floor can easily be moved by children, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, a gate will be placed at stairs secured to the railing, a photo of the correction will be sent to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above in that dishwashing pods and lysol aerosol were on counter of kitchen, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee removed the items from the counter and cabinet during visit. Per licensee, cleaning items will be placed in secured cabinets and the locks will be checked regularly to make sure they are still wroking.
Based on observation, the licensee did not comply with the section cited above. LPA observed and photograph cleaning products and personal hygiene products under the kitchen and bathroom sink and was accessible to children, which poses a potential health, safety risk to persons in care.
Resolution: LPA observed during inspection, Licensee remove some items. Per Licensee, was remove all items and placed them in the garage or buy latches and send LPA proof via email by POC due date of 10/11/22.
Based on observation and interview, the licensee did not comply with the section cited above. Per Licensee was not aware that CPR & 1st Aid training needed to be completed every two years, which poses posed a potential health, safety risk to persons in care.
Resolution: Per Licensee will take an in person CPR & 1st Aid class and send copies of the certificate via email by POC due date of 10/28/22.
