Based on record review, the licensee did not comply with the section cited above as 3 of 4 children's files reviewed by LPA are not complete. One child in attendance during the inspection does not have a file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with a list of missing documents. Licensee will obtain missing documents and file in children's file. Missing documents for the 4 children will be submitted to LPA via email at jialing.zhu@dss.ca.gov by 07/31/2025. LPA provided Licensee with a list of documents to keep as a resource. Moving forward, Licensee will maintain complete files for all children enrolled at the facility.
Based on LPA observation and record review, the licensee did not comply with the section cited above as Licensee was the only adult present in the home with 8 preschool-age children upon LPA arrival, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: One child in attendance during today's inspection will no longer be attending the facility after today. LPA discussed ratio requirements with Licensee. A poster illustrating ratio requirements was provided to Licensee as reference. This citatio will be cleared at the end of the inspection on 07/16/2025.
