Based on record review, the licensee did not comply with the section cited above as assistant did not have proof of immunizations for measles (MMR), pertussis (Tdap), and flu on file. Assistant also does not have negative TB test results on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant will obtain proof of immunizations for measles (MMR), pertussis (Tdap), and flu (seasonal, can be declined with written statement. Assistant will also obtain negative TB test results that are no older than 12 months of her start date or from after the start of employment at the facility. Licensee will send proof of immunization and TB test results OR scheduled appointment(s) to LPA via email at jialing.zhu@dss.ca.gov before or on 10/31/2025.

Based on observation, the licensee did not comply with the section cited above as multiple bottles of disinfectant spray (Lysol) and disinfectant wipes were left accessible to children in the on-limits bathroom on top and side of the toilet, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee made the disinfectant sprays and wipes inaccessible to children during the inspection, thus the deficiency has been corrected. Excess containers of soaps and hand sanitizers were also moved and made inaccessible to children.

Resolution: Licensee will thoroughly clean the toys and play equipment in the outdoor play area and remove any broken equipment from the on-limit outdoor play areas. Licensee will submit photo(s) of cleaned toys and equipment and a written statement to confirm removal of broken equipment to LPA at jialing.zhu@dss.ca.gov.
