Based on record review, the licensee did not comply with the section cited above as Licensee and assistant does not have current Mandated Reporter (Child Care Providers) training certificates, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant will complate the Mandated Reporter (Child Care Providers) training on mandatedreporterca.com. Licensee will send both certificates to LPA via email at jialing.zhu@dss.ca.gov before or on 10/31/2025.

Based on record review, the licensee did not comply with the section cited above as assistant did not have proof of immunizations for measles (MMR), pertussis (Tdap), and flu on file. Assistant also does not have negative TB test results on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant will obtain proof of immunizations for measles (MMR), pertussis (Tdap), and flu (seasonal, can be declined with written statement. Assistant will also obtain negative TB test results that are no older than 12 months of her start date or from after the start of employment at the facility. Licensee will send proof of immunization and TB test results OR scheduled appointment(s) to LPA via email at jialing.zhu@dss.ca.gov before or on 10/31/2025.

Based on observation, the licensee did not comply with the section cited above as multiple bottles of disinfectant spray (Lysol) and disinfectant wipes were left accessible to children in the on-limits bathroom on top and side of the toilet, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee made the disinfectant sprays and wipes inaccessible to children during the inspection, thus the deficiency has been corrected. Excess containers of soaps and hand sanitizers were also moved and made inaccessible to children.

Based on observation, the licensee did not comply with the section cited above as toys and play equipment (tricycles, bikes) are ample but not clean due to presence of spider webs and dirt. One (1) tricycle did not have pedals and the handle bars and pedal bars are exposed, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will thoroughly clean the toys and play equipment in the outdoor play area and remove any broken equipment from the on-limit outdoor play areas. Licensee will submit photo(s) of cleaned toys and equipment and a written statement to confirm removal of broken equipment to LPA at jialing.zhu@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in that current Mandated Reporter certificates (child care providers AB1207) was not completed and/or done for licensee and assistant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant to go to www.mandatedreporterca.com and complete the Child Care Providers AB1207. Going forward, this certificate must be renewed every two years.

Based on record review, the licensee did not comply with the section cited above in that licensee's CPR/1st Aid training expired on January 13, 2023. No current certificate available for assistant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant to complete CPR/1st Aid training. Going forward, this certificate must be renewed every two years.

Based on record review, the licensee did not comply with the section cited above in that licensee and assistant did not have proof of pertussis vaccination or a booster in their personal file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant provide proof or tdap (or pertussis) vaccination or a booster shot.

Based on file review, the licensee did not comply with the section cited above in that two of six children's files reviewed found they were missing the signed proof of receipt for the LIC995A Parent's Rights form which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to collect signed proof of receipt for the LIC995A Parents Rights form.

Based on record review, the licensee did not comply with the section cited above in that the last disaster drill was completed on August 12, 2020 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to conduct a disaster drill (fire or earthquake) with the children. Going forward licensee to complete a drill at lease once every six months.
