Based on observation, the licensee did not comply with the section cited above as stairs in the main house that leads to the second floor were not barricaded in any way and a children under the age of 5 was in the main house during the inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will probably barricade the bottom of the stairs. Licensee will provide proof of correction by sending a photo of the stairs after it has been barricaded to LPA via email at jialing.zhu@dss.ca.gov by 06/02/2025. Until the stairs are barricaded, children will not be permitted in the main house as a safety precaution.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee stated 15-minute sleep checks were not conducted regularly and documented, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin conducting and documenting 15-minute sleep checks for all infants 0-24 months of age. LPA provided Licensee two different templates she may use, or she can use her own method as long as Licensing requirements are met. Licensee will email three days worth of documention to LPA at jialing.zhu@dss.ca.gov by 06/02/2025.
