Based on interview and record review, the licensee did not comply with the section cited above as two (2) infants in care did not have 15-minute sleep checks documented, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct and document 15 minute sleep checks for all infants (ages 0-24 months). Licensee will at least 3 days of documented sleep checks for each infant to LPA via email at jialing.zhu@dss.ca.gov by 05/02/2025.

Based on observations and record review, the licensee did not comply with the section cited above as 14 children were present during today's inspection. One child was not at least 6 years of age as required by Licensing regulations, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will either unenroll one child or stagger the schedule of children so that ratio can be maintained. Licensee will either provide a written statement from parent of child verifying their child is unerolled or Licensee will provide a written statement and schedule of children that ensures ratio will be maintained. Licensee will send the above information/documents to LPA at jialing.zhu@dss.ca.gov by 05/02/2025.
