LPA met with Director Veronica Leon. Also present during the inspection were five (5) fingerprint-cleared staff and 19 children in care. The facility is within ratio and in compliance with capacity regulations today. The facility was toured to conduct a Health and Safety Inspection. The incident occurred on 10/16/2024 and was reported to Licensing on 10/17/2024. On 10/16/2024 at approximately 10am, one (1) child was outside in the play yard unsupervised for approximately 2 minutes during transition time. A classroom teacher noticed the child was still outside, and the child was brought into the classroom immediately. During today's inspection, LPA reviewed the above incident with Director and conducted an interview with the Director to obtain more information. LPA observed the play yard to be fully fenced and secured. The classroom door was open during the incident. LPA observed teachers are able to have direct line of vision of the play yard when looking through the classroom door from the inside. However, the teacher's back was turned away from the door during the incident. Based on the obtained information and LPA’s observations, LPA determined there was no visual supervision of the child in the play yard, which posed an immediate threat to the health and safety of the child in care. A Type A deficiency is being issued during today's inspection. Director was notified of this incident the day of the incident. Director notified her supervisor. Director spoke with the teachers involved in this incident in the evening. An all staff meeting was held the following day to discuss the incident and how future similar incidents can be prevented. Director reviewed supervision guidelines with staff. Director notified parents of the child over the phone and had an in-person discussion the day after the incident. As a result of the Type A deficiency cited during today’s inspection, a copy of this report and LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to parents/guardians of current enrolled children in care and all children enrolled following a 12-month period of this report. The LIC 9224 must be signed within the next business day the children are in care and is to be kept in the children’s files. A Notice of Site Visit was given and must remain posted for 30 days.
