Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed one staff member did not have LIC 503 (health screening). Director Stated that it was completed but does not know what happen to it.

Based on record review, the licensee did not comply with the section cited aboveas 2 of 5 staff/volunteer files reviewed by LPA did not have TB test results, which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed 4 staff did not have a current mandated reporter training. Director stated that all staff members are currently in the process of completing their mandated reporter training.
