During today, LPA observed 10 children file, however pre licensee's sign in/out sheet, there are 7 children wthout file and all the LIC paperwork, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will email all 7 children information to the LPA by 5/2/2022
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. LPA observer several children filds and without IZ record, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will get the IZ record from the parents and fill out Blie IZ card and email to the LPA.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. On 4/25/2022, LPA arrived around 8:02 am. No one answer the door. Arond 8:09, A lady (Jacqueline M.) arrived. A assistant (Patricia M,) opened the door and there are 10 children inside the daycare. which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: After Jacqueline M (assistant) arrived @ 8:09 am, the teacher/child ratio was good. The licensee will talk to her assistant regarding the capacity regulations.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. The licensee does not renewal her mandated reporter training ,which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will email her certification to LPA by 4/27/2022.
