Per record review and interviews on November 17,2025, C1 was left without the supervision of an adult which threatened the emotional health of C1, this unusual incident was not reported to the Department as required. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above. LPA talked to the director, Susan Gaines. According to the director, the facility is scheduleing to the lead water testing soon and the facility will have water deliver service, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in when five instant cold pack, anti septic solution and burn cream were located in a drawer that was accessible to the children in care, which poses/posed a potential health, safety or personal rights risk to persons in care.
