When LPA arrived to the facility, Elsa Godinez was helping the licensee with thouse LiveScan and other documentations, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Elsa left the facility. LPA told the Licensee the requirement for Criminal Record Clearances and quality to be an assistant

Resolution: The Licensee will start login the infant's sleep and share this information the her assistant
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above, LPA observed 5 children file and 3 of the children do not have Immunized record, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee will ask parent to bring the children's shot record.
