Based on observation, the licensee did not comply with the section cited above. LPAs observed 3 infants with blankets,toys, and diapers in their playpen while they are sleeping, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee shall ensure no blankets,toys, etc are in the playpen while infants are sleeping.
Based on observation, , the licensee did not comply with the section cited above. LPAs observed the right side of the wrought iron latch unable to self-latch and self-close, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The right self-latch on the right side of the home shall be fixed to ensure the latch self-closes, self-latches, and is no more than 6 inches from the top of the wrought iron gate. Backyard will remain off-limits until latch is fixed.
Based on observation and record review,, the licensee did not comply with the section cited above. LPAs observed a total off 11 day care children (3 infants, 7 preschoolers, and 1 school age child) in care without a qualified assistant present, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Assistant arrived about 15 minutes after LPAs arrived. Licensee shall ensure she reverts back to a small family child care ratio when she does not have an assistant present.

Resolution: The license will get a new fire extinguisher or get the service. She will email the receipt to LPA.

Resolution: The licensee will clean and organize the kitchen area and remove medicines, cleaning compounds and unsafe items and email to LPA.
