Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above where the key lockable device is lower than 60" from the ground. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will correct the latch and move to at or above 60" by POC date.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above where the rescue pole was noted to be 9' in lengh which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states they will ensure a 12' pole with rescue hook is maintained at the facility and will provide proof of such to LPA by POC date.

Based on observation, the licensee did not comply with the section cited above there was dog feces in the back yard, dog pee pee mat in the kitchen area and broken glass on the ground of the patio. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: assistant cleaned the glass at the time of inspection. Licensee will send photos to LPA via text or email of dog feces cleaned from backyard by 8/20/24.
Based on observation and interview, the licensee did not comply with the section cited above the pool gate was not self latching and LPA was informed the children use the patio to get fresh air during the day. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will have the gate fixed and send LPA Flores a video of the door self latching by 8/27/24 via text or email.
Based on observation, the licensee did not comply with the section cited above the carbon monoxide detector was broken at the time of inspection. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee ordered a new carbon monoxide detector at the time of inspection, stated it will be delivered by 8/16/24. Licensee will submit proof of correction, video to be sent to LPA Flores via text or email by 8/21/24.
Based on LPA observation, the licensee did not comply with the section cited above, bedroom doors were not closed at the time of inspection, the school age bathroom the children utilize had hygiene items, cleaning products, bedroom the children sleep in had precription medication on the shelf above the bed, hygiene items on the dresser (bedroom belongs to the grandaughter, also utilized as a sleeping room. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee had the medication removed right away, the licensee will have child safety latches placed in the bathroom and cleaning products were removed during inspection. Licensee will send LPA Flores via email or text by 8/27/24.
Based on interview and record review, the licensee did not comply with the section cited above the licensee has not conducted a drill since October of 2023. This poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in the post that holds the self latching gate was wobbling and needs to be fixed to allow the gate to be able to self latch. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee does not allow the children in care to go into this side of the yard it is separately gated from the children's outdoor play yard. The licensee called a repairman to come and see what is needed to fix the gate on 12/22/23. Licensee will send the LPA photos of self latching gate and post fixed by 1/04/24 via emai.
Based on observation, the licensee did not comply with the section cited above in the children's outside play area had dog feces, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have the area cleaned up and send LPA Flores photos of backyard clean for children to play in. Licensee stated at time of inspection, the outdoor children's play yard has not been used for the past couple of weeks due to low enrollment of children and the holiday activities. Licenee will send proof of correction to LPA Flores by 1/04/24.
Based on record review, the licensee did not comply with the section cited above in staff #2 did not have proof of immunizations on file at time of inspection, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain proof of immunizations for staff #2 and send photo to LPA Flores via email by 01/04/24.
