Based on observation & interview, the licensee did not comply with the section cited above in in room #3 children were having diapers changed in the middle of the classroom, with the nearest sink at least 20 feet away, which poses a potential health, safety or personal rights risk to persons in care.

Based on interviews and observations conducted on 02/14/22, LPAs observed a total of 29 children who were indoors and was not wearing a mask inside the classrooms.

Based on observation, the licensee did not comply with the section cited above in the playground has two items that have chipped paint at time of inspection. which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in four out of five staff ddid not have a currently mandated reporter ceriticate at the timeof inspeciton, which poses a potential health, safety or personal rights risk to persons in care.

Based on the Director's own admission via email correspondence, the Licensee did not notify parents/authorized representatives of the third positive COVID-19 case that was reported to the facility on 02/08/2022, which poses a potential health, safety or personal rights risk to persons in care.
