Based on observation, the licensee did not comply with the section cited above. The bathroom used by children possesed clearning agent at reach of the children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a child safety lock to ensure the children cannot acess the cleaning products. Licensee will comply to this by the date stated above and send pictures to LPA to verify this has been completed

Based on record review, the licensee did not comply with the section cited above. Mandated reporter certifcate was not present in there records, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the mandated reporter training by the date stated above and will send a copy of the certificate to LPA

Based on record review, the licensee did not comply with the section cited above as licensee was checking on infants but no documentation was being done which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will start documenting the fifteen minute checks and submit a picute to LPA Rios by 12/5/2022 of sleeping chart. Picture will be sent by text or email to either 805 635 4725 or austin.rios@dss.ca.gov

Based on observation, the licensee did not comply with the section cited above in [count] of the door being closed of the room infant was sleeping in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee corrected the violation during the visit as she opened the door and was told that the door to the rooms infant sleeps in must remain opened at all times and licensee has to be able to observe the children. Licensees stated the door will remain open now.
