Resolution: Both Licensee and Assistant will complete Madated Reporter Training and will email or text certificate to LPA cynthia.alvarez@dss.ca. gov or 805-689-4212

Resolution: Licensee will provide proof of required immunizations for their Assistant and will l email or text immunizations to LPA cynthia.alvarez@dss.ca. gov or 805-689-4212

(D) Documentation shall be maintained in the infant's file and be available to the Department for review. Documentation shall include the following: a. Date. b. Infant's name. c. Time of each 15-minute check.
Resolution: Licensee was provided a log and will be required to submit completed log by the date indicated above, licensee will email or text completed logs to LPA cynthia.alvarez@dss.ca. gov or 805-689-4212
