Based on interview and record review, the licensee did not comply with the section cited above which licensee had an expired CPR training and did not have valid training certification which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to sign up for CPR training, take the training and submit certification to LPA.

Based on interview, the licensee did not comply with the section cited above in which licensee was not documenting 15 minute safe sleep checks for infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to begin documenting safe sleep checks and send LPA two weeks worth of safe sleep checks.

Based on interview, the licensee did not comply with the section cited above in which licensee did not have a valid mandated reporter certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee has already scheduled mandated reporter training for 3 August 2024. Licensee is to send mandated reporter training certificate to LPA.

Based on interview and record review, the licensee did not comply with the section cited above in which licensee was not documenting disaster drills which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to conduct a disaster drill, document the drills, and then send documentation to LPA.

Based on record review the licensee did not comply with the section cited above in as CPR certifcate expired in July 2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licesee will submit proof of CPR Card to LPA Rios by phone at 805 635 4725 or by email at austin.rios@dss.ca.gov by 1/13/2023

Based on interview and record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licesee will submit proof of LIC 9227 infant safe sleep form to LPA Rios by phone at 805 635 4725 or by email at austin.rios@dss.ca.gov by 12/22/2022

Based on interview and record review the licensee did not comply with the section cited above in as there was no documentation on file for the infant enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licesee will submit proof of 15 minute documentation while infant is sleeping to LPA Rios by phone at 805 635 4725 or by email at austin.rios@dss.ca.gov by 12/22/2022
