Based on interview and record review the licensee did not comply with the section cited above in which LPA asked licensee if they were conducting safe sleep documenation and licensee said they were not which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Document 15 minute safe sleep checks for the infants in care for two weeks. Send documentation to LPA through email within two weeks. If infants in care do not sleep, licensee if to write a note for weeks weeks stating the time the infant was dropped off and left and that they did not sleep. They must sign and the parent must sign.
