Based on observation the licensee did not comply with the section cited above LPAs observed medication and cleaning agents accessible to children which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee made the areas inaccessible with a child safety gate during the time of inspection with LPAs present. LPAs reminded licensee to keep off-limits areas inaccessible to children. LPAs is requesting licensee to attended and in office compliance plan with LPAs and LPM.
Based on observation the licensee did not comply with the section cited above LPAs observed gas can outside accessible to children which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee made the areas inaccessible with a child safety gate during the time of inspection with LPAs. LPAs reminded licensee to keep off-limits areas inaccessible to children. LPAs is requesting licensee to attended and in office compliance plan with LPAs and LPM.
Based on interview and record review, the licensee did not comply with the section cited above licensee stated they are conducting 15 minutes checks however are not logging the 15 minute sleep checks which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will document 15 minute sleep checks for infants under 24 months and submit copies to LPA by 3/5/36.
Based on record review, the licensee did not comply with the section cited above the licensee mandated reporter training is expired and assistant does not have the required mandated reporter training for child care provider which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit copy of the licensee and assistants mandated reporter training by 3/5/26.
Based on record review, the licensee did not comply with the section cited above licensee was unable to provide assistants required immunization or TB test which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit copy of assistant required immunization and TB test by 3/12/26
