BERNAL FAMILY CHILD CARE

Based on (observation), the licensee did not comply with the section cited above in the presence of an excluded person who has not obtained a criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During inspection unauthorized indvidual left the facility. Licensee stated that the individual will not return until a criminal record clearance is received.

Based on (observation) and (record review), the licensee did not comply with the section cited above in not documenting fire and disaster drills every six months which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire and disaster drill and document the date and time. Licensee will send a copy to LPA via email ensuring documentation is being done

Based on (observation) and (record review), the licensee did not comply with the section cited above in Licensee was not documenting infants sleep log every 15 mins which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin documenting infants sleep log every 15 mins as of today and will send LPA a copy of sleep log via email Fernando.hernandez@dss.ca.gov

Based on (observation) and (record review), the licensee did not comply with the section cited above in Licensee and assistant did not have a current Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send LPA a confirmation date for Mandated Reporter Training and will send certificates for Licensee and Assistant via email

Based on (observation) and (record review), the licensee did not comply with the section cited above in assistant did not have vaccinations on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will acquire a copy of assistants immunizations and send to LPAs email Fernando.hernandez@dss.ca.gov

Based on LPA record review and licensee interview, the licensee did not have proof of purchase or service tag for the fire extinguisher which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof purchase or service for the fire extinguisher to LPA Cervantes via email, fax, or by mail by 06/02/2022.

Based on LPA record review and licensee interview, the licensee did not complete Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of Mandated Reported Training to LPA Cervantes via email, fax, or by mail by 06/02/2022.

Based on LPA record review and licensee interview, the licensee did not renew Pediatric First Aid/CPR training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of Pediatric First Aid/CPR certification to LPA Cervantes via email, fax, or by mail by 06/02/2022.

Based on LPA observation and licensee interview, licensee did not meet the criteria for staffing ratio since licensee was providing care to 8 children under the age of 4 years. This poses an immediate risk to the health and Safety of children in care.

Resolution: Licensee agreed to submit a written plan of correction stating how she will ensure that the ratio and capacity are being followed at all times to Licensing for review by 05/20/2022. Licensee will be in advise two parents in care to seek alternate childcare in order to be in compliance.
