Based on observation, the licensee did not comply with the section cited above in 3 counts out of children in care sleeping with items in the pack in play which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately took out items from the pack and plays. Licensee to watch safe sleep video on CCL website and follow up with a signed statement that licensee watched the video along with what the Licensee learned. Licensee also will have all parents sign LIC 9224 and all future enrolled children for 12 months.
Based on record review, the licensee did not comply with the section cited above in 2 out of 3 files which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Assistants to complete mandated reporter training by 10/10/23 and send certificate to LPA Sims via email
