Based on record review, the licensee did not comply with the section cited above in one (C2) out of one infant in care missing 15 min sleep checks which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to document three days worth of naps for C2 and send documentation to LPA by 2/13/26. Send sleep checks to LPA at tammy.dutra@dss.ca.gov.
Based on record review, the licensee did not comply with the section cited above in 1 out of 3 children did not have immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licnesee agrees to obtian a copy of C3's immunization records and submit a copy of them to LPA Danielson by 03/01/2025 via email at kayla.danielson@dss.ca.gov.

Resolution: Licensee and Assistant will complete mandated reporter training by 2/26/24 and send copy of certificate to LPA Sims by 2/26/24 to email sydney.sims@dss.ca.gov
