Resolution: Licensee agrees to conduct a fire drill and document on a fire drill log and send to CCL by 12/4/24. Send fire drill log to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in both Licensee and assistant missing updated mandated reporter certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to take mandated reporter training and have her assistant take mandated reporter training and send updated mandated reporter certificates to CCL by 12/4/24. Send certificates to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in assistant missing immunization record (MMR/ TDAP and TB test) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get a copy of assistants immunization record and TB test to CCL by 12/4/24. (A titer may be performed if necessary to prove immunity.) Send copy of immunization record to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in two out of four children's files missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get copies of C3 and C4's immunization records and send to CCL by 12/4/24. Send immunization records to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in two out of two children's files missing LIC 627-Consent for medical treatmentwhich poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to send a copy of LIC 627 to CCL by 12/4/24. Send LIC 627 for C3 and C4 to LPA at tammy.dutra@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above, having posions accessible to children in unlocked shed and cleaning compunds accessible to children underneath bathroom sink which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately locked shed and removed cleaning supplies. Licensee will write statement saying that the understand posions and cleaners need to be stored correctly and send statement to LPA Sims via email to sydney.sims@dss.ca.gov. Licensee will have all enrolled children's parents sign LIC 9224 and all childrens' parents who enroll for the next 12 months. LIC 9224 must be maintained in childrens files

Based on record review, the licensee did not comply with the section cited above in 1 count of not documenting 15 minute checks for infant while sleeping which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will document 15 minute checks for infants while in care and provide copy of documentation to LPA Sims via email to sydney.sims@dss.ca.gov by 12/01/2023. Licensee will have all enrolled children's parents sign LIC 9224 and all childrens' parents who enroll for the next 12 months. LIS 9224 must be maintained in childrens files
