Based on record review, the licensee did not comply with the section cited above in Licensee was missing updated mandated reporter certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to take mandated reporter training and send certificate to CCL by 12/4/24. Send certificate to LPA at tammy.dutra@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above in gate that prevents children from accessing the barn and off limit areas that contains an animal water trough and farm equipment was left unlocked and open which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to close and lock gate leading to the off limit areas of the yard and send a picture to CCL by 11/26/24. Send picture of locked gate to LPA at tammy.dutra@dss.ca.gov. Licensee will also have parents sign LIC 9224 and return to CCL by the next day they are in care. Licensee needs to keep LIC 9224 in children's files for review.

Based on record review, the licensee did not comply with the section cited above in the Licensee is missing a fire drill log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire drill and send a picture of the log to CCL by 12/4/24. Send picture of fire drill log to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in three out of three childrens files reviewed were missing LIC 700 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have three children's parents fill out LIC 700 and send to CCL by 12/4/24. Send copies of LIC 700 to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above, Licensee not documenting the 15 minute checks after they are being conducted which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send copy of Safe sleep logs to LPA Sims by 1/13/23 for all infants that are in care via email to sydney.sims@dss.ca.gov.
