Resolution: Licensee agreed to submit the plan of correction letter outlining how to ensure that immunization will be obtained prior to admission of a day care childe. Submit no later thatn 3/17/2025.
Based on observation, the licensee did not comply with the section cited above , Fire Extinguisher was purchased on 12/29/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to purchase a new regulation fire extinguisher and send the receipt no later thatn 3/17/2025.
Based on observation and interview, the licensee did not comply with the section cited above, Licensee stated she recently purchased the fire extinguisher however receipt was not available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to provide proof of purchase of the fire extinguiser no later thatn 2/14/2022 to show that it was pruchse within the 1 year period.
Based on record reviewthe licensee did not comply with the section cited above Licensee's CPR and First Aid expired on 3/302021 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to renew the Pediatric CPR and First Aid Certificate and submit the proof no later thatn 2/21/2022.
