Based on observation, the licensee did not comply with the section cited abovewhich poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit plan to make pool inaccessible to LPA by POC due date. Licensee will make the pool inaccessible to children in care within 7 days.

Based on observation, interview, and record review the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensees will send proof of complete Mandated Reporter Training for both licensees to LPA Piring by 4pm on POC due date.

Based on observation, the licensee did not comply with the section cited above in one out of one sleeping infant, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: L2 stated she will not swaddle any infant during hours of care and will instead place the infant on their back in a crib or play yard with no blankets or other loose articles. Based on observation, the licensee did not comply with the section cited above in one out of one sleeping infant in car, which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in seven out of seven children, which poses/posed a potential health, safety or personal rigths to persons in care.

Resolution: L2 stated she will obtain immunization records and complete blue cards (CDPH 286) for all children in care. L2 stated she will provide evidence of this to LPA via email by POC due date.
