Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 staff member. Assistant did not have current Mandated Reporter training certificate, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will submit current Mandated Reporter training certificate for the assistant to LPA by due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in two out of two infants' files in care. Licensee did not have LIC 9227 for the two infants in care, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will submit the LIC 9227 for the two infants in care: Child #3 and Child #9, to LPA by due date.
Based on record review, the licensee did not comply with the section cited above. Licensee has a valid infant/child CPR & First Aid certificate with expiration date of 4/28/20 from National CPR Foundations, however it does not have the EMSA sticker. This posed a potential health and safety risk to persons in care.
Resolution: Licensee will register to take an EMSA approved infant/children CPR/first aid course by due date. Licensee will provide a copy of the certificate to LPA via email.
