Based on interview and record review, the licensee did not comply with the section cited above in that, licensees stated that they do not have current mandated reporter training certificates, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee Sarah Holtz stated that they will submit update mandated reporter training certificates to LPA via email by due date.
Based on interview and record review, the licensee did not comply with the section cited above in that, licensee did not maintain a Sleep Log for infants (children 24 months and under), which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee Sarah Holtz stated that they will submit to LPA copy of current Sleep Log for one infant enrolled via email by due date. Licensee stated that will provide Sleep Log for dates 3/26/25-3/28/25 to LPA for review and maintain Sleep Logs for infants going forward.
Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 staff members, both staff members did not have current EMSA approved Pediatric CPR and First Aud certificate, which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states states she will submit current/updated EMSA approved Pediatric CPR and First Aid certificates to LPA by due date.
