It was reported by Tammy L. Hoover, Director, and Jamie L. Michaels, Owner/Licensee. LPA Castro explained the reason for today's visit. Director led LPA on a tour of the facility. During today's inspection a tour of the facility was conducted and census was taken of the preschool children: LPA observed 11 children with 2 teachers in room 4, 13 children and 3 staff outside playing from room 3 and 5 children and 1 teacher from room 5, also playing outside in another area. The facility was observed operating within its licensed capacity and within compliance of staff to child ratios. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances. During today's inspection, staff members were interviewed. Three staff interviewed stated that (Staff 1) S1 had spoken to children in a sharp tone. One staff interviewed had witnessed S1 yanking (Child 1) C1 and forcefully sitting child down. Deficiency regarding discipline was found. Director and Owner/Licensee had previously placed S1 on a corrective action plan leading to termination. S1 is no longer employed at the facility. This termination occurred on Tuesday, December 17th, 2024. Continue to next page. The deficiency is being cited on the attached 809D. Due to the Type 'A' violation the licensee shall post and provide copies of the report to parents/guardians of the children in care at the facility by the next business day and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. In addition, the licensee shall immediately post upon receipt the Proof of Correction for 30 consecutive days. The licensee is to keep Acknowledgement Receipt (LIC9224) signed by parents in each child’s file. Appeal Rights and deficiency was discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Tammy Hoover and Owner/Licensee Jamie Michaels.
