Based on LPA observation and record review, the licensee did not comply with the section cited above in 1 out of 10 which poses an immediate health, safety or personal rights risk to persons in care. Office staff was not listed on the Guardian Facility Roster. The Director stated she received a letter from the Department indicating the employee had been cleared; however, it was not the correct form. Director was under the impression that the employee had been cleared by the Department and that no further action was required on their part. Employee has been working since Nov. 2024.
