During the time of arrival, LPA noted the assistant parking her car, then getting out of her car and go into the home. Upon arrival, licensee was not home, a home resident was, who was caring for 10 children while licensee and assistant were out of the home. Furthermore, home resident did not meet all required documentation. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a written statement of understanding of ratios and capacity. LPA provided the licensee with the following regulation to read over and send an understanding of: Staffing Ratio and Capacity: 102416.5
Based on record review and licensees own admission, the licensee did not comply with the section cited above. Per licensee, although S2 does not always provide care for children in care and/or assist, S2 was left alone with 10 children in care. S2 does not have an updated Mandated Reporter Training (MRT) Certificate for review. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of Mandated Reporter Training (MRT) for S2, no later than the POC due date. The free training can be found online at mandatedreporterca.com The training should be done for "Child Care Providers".
Based on record review and licensees own admission, the licensee did not comply with the section cited above. S2 did not have immunizations (measles(MMR), pertussis(Tdap)) on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee is asked to submit proof of immunizations for S2 no later than the POC due date. Immunizations due are: proof of measles (MMR), pertussis (Tdap) and flu shot; or written declination of flu shot with S2's signature and date.
Based on LPA observations; the on-limits restroom, had 3 cleaning solutions (1 gallon, 1 half gallon, and 1 spray). Furthermore, the kitchen is on-limits, where LPA found 4 multi-cleaner sprays). There was also a cleaning spray found on the kitchen counter, near snacks and food. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to remove items and sent LPA pictures of under the restroom sink as well as kitchen sink and kitchen counter. This is due to LPA no later than the POC due date.
Based on LPA observation, the sheet the infant was sleeping on was not a fitted sheet. Although this typically poses an immediate threat to the health and safety of children, the infant is 19 months, downgrading this to a Type B.This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to send pictures of the front and back of the crib mattress with a fitted sheet to licensing no later than POC due date.
Based on record review and licensees own admission, the licensee did not comply with the section cited above. 1 out of 2 staff did not have their immunizations on file; MMR(measles), Tdap(pertussis) and/or Flu shot/written declination were not on file during todays inspection. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to provide proof of immunizations: MMR (Measles), Tdap (Pertussis) and/or Flu shot/written declination, signed and dated for S1 to the department, no later than the POC due date.
Based on record review, the licensee did not comply with the section cited above. 1 out of 2 staff did not have an updated Mandated Reporter Training (MRT) on file. Although licensee was able to provide proof of "general" training, the one for "Child Care Providers" has expired. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of completion of "Child Care Provider Training" certificate to the department no later than the POC due date.
