Based on LPAs observation, there was a nail kit with sharp utensils in it, as well as a pack of blades in the on-limits restroom medicine cabinet. Facility representative removed immediately& no children were observed in the restroom at the time of observation, therefore this is being downgraded to a Type B. This poses/posed a potential health, safety/personal rights risk to persons in care.
Resolution: Facility representative was able to remove immediately, making these items inaccessible to children in care.
Based on record review, although a 15 minute sleep log was provided for the infant in care during todays inspection, the times checked did not reflect every 15 minutes. Some were every 20 minutes, and some were more. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility representative agrees to submit 15 minute sleep logs for C1 to the department, no later than the POC due date.
Based on observation the licensee did not comply with the section cited above in that at 11:45AM LPA observed cleaners and bathroom products accessible to children within the bathroom which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will submit proof of making cleaners and bathroom products inaccessible to childern in care by POC due date to the department.
Based on [(observation) (interview) the licensee did not comply with the section cited above in that stairs do not have gate at bottom of stairwell but at entry to main daycare room. LPA observed S1 and their 2 children (under 5) accsss stairwell and children passing stairs with staff for pick up and drop off which poses/posed a potential health, safety or personal rights risk to persons in care. Per staff interviews, did not know regualtion applied to their own children.
Resolution: Facility will provide addtional gate at bottom of stairwell by POC due date. LPA reviewed regulation with Assistant and Licensee that regulation applies to all children under 5 during hours of operation.
Based on (record review)], the licensee did not comply with the section cited above in that last drill record available for review was 10/26/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will complete fire drill by POC due date and send proof (video) to the department.
Based on interview and (record review)], the licensee did not comply with the section cited above in that facilty is not using safe sleep logs for 15 minute checks which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will complete 15 minute safe sleep logs for all infants enrolled by POC due date and submit proof of logs to the department. 15 minute safe sleep log provided to Licensee
Based on (record review)], the licensee did not comply with the section cited above in that S1 did not have mandated reporter available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will submit mandated reporter certificate for S1 to the department by POC due date or earlier.
Based on [(observation) and record reveiw , the licensee did not comply with the section cited above in that at 11:48AM LPA observed 3 infants and one preschool in highchairs in the kitchen area that is marked off limits which poses/posed a potential health, safety or personal rights risk to persons in care. Facility sketch also shows kitchen area as off limits.
Resolution: Children removed from off limit area to main day care room. Cleared during this visit.
Based on [(observation) and (record review)], the licensee did not comply with the section cited above in that 4 out of 5 files reviewed had missing immunizations (C2-C5) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will submit proof immunizaions for C2-C5 by POC due date or earlier to the department.
Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file review LPA was unable to review sleep logs for C2. Licensee stated she stopped doing sleep logs.
Resolution: Licensee understands per Safe Sleep regulations all children up to the age of 24 months must be checked every 15 minutes while sleeping and observation must be documented on a sleep log. Safe Sleep regulations were provided during inspection.
Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the record review the LPA was unable to review current Mandated Reporter certificates foe the Licensee and her Assistant.
Resolution: Licensee agrees to complete Mandated Reporter training and submit the certificate of completion to the Department for herself and her Assistant on or by the POC due date. Licensee understands that Mandated Reporter must be completed every two years. Training can be completed at www.mandatedreporter.com
Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the children’s file review LPA was unable to review immunization records for C2, C4 and C5. Licensee stated she does not believe the Authorized Representatives gave her the records during enrollment.
Resolution: Licensee agrees to obtain and submit immunizations to the Department for C2, C4 and C5 on or by POC due date. Licensee understands that all children enrolled must have immunizations available for review during inspection.
Based on the observation the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the facility tour the LPA observed a bunched up blanket near the feet of a sleeping infant in a playpen. During the facility tour the Licensee took the blanket and covered the sleeping infant.
Resolution: Licensee understands per Safe Sleep regulations there shall not be any loose items in the crib/playpen/bassinet. Safe Sleep regulations were provided during inspection. Licensee agrees to read Safe Sleep regulations and submit a written statement of understanding.
Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the record review LPA was only able to review a fire drill inspected 9-8-2022. Licensee stated that was the only fire drill they conducted and no other records are available for review.
Resolution: Licensee understands fire drills must be conducted every six months per Title 22 regulations. Licensee agrees to read Operation of a Family Child Care regulation as it pertains to fire drills and submit a written statement of understanding.
Based on the observation the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the facility tour the LPA observed a loosen blanket in place of a tight fitting sheet.
Resolution: Licensee understands per Safe Sleep regulations there shall not be a tight fitting sheet on the mattress. Licensee agrees to read Safe Sleep regulations and submit a written statement of understanding.
