TURCAZ FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above. During todays visit, there was an infant sleeping in a crib with a blanket, pillow, bottle and 2 blankets underneath him. in This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to hold a meeting with her assistants about safe sleep. Licensee agrees to submit the following to the department no later than the POC due date: proof of an agenda with licensees signatures as well as both her assistants.

Based on record review and licensees own admission, the licensee did not comply with the section cited above. 3 out of 3 staff did not have their Mandated Reporter Training (MRT) on file. All 3 certificates expired 08/2025. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of Mandated Reporter Training (MRT) for S1, S2 and S3 to the department no later than POC due date. This training is free, and can be found online at mandatedreporterca.com- and must be done for "Child Care Providers".

Based on record review, the licensee did not comply with the section cited above. There were missing staff immunizations during todays visit. Missing MMR(measles) for S1 and S2. Missing Tdap for S2, and missing TB for S3. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit the above missing items to the department no later than the POC due date. Missing MMR for S1 and S2. Missing Tdap for S2, and missing TB for S3.

Based on observation and record review, the licensee did not comply with the section cited above in that safe sleep logs are not used for 15 minute checks which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility will submit completed safe sleep logs (pictures) to the department by POC due date.

Based on observation the licensee did not comply with the section cited above in that LPAs observed 2 baby walkers on site for infants present which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Facility placed baby walkers in off limit area (garage) and per assistant to be sent home with parents.

Based on [(observation) the licensee did not comply with the section cited above in that LPAs observed infants with blankets, bottles, pillows while sleeping in play yards which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Facility staff removed all loose items from play yards during this visit.

Based on [(observation), the licensee did not comply with the section cited above in that LPAs observed cleaners and bleach under kitchen and bathroom sink which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility staff removed cleaners during this visit.

Based on [(observation), the licensee did not comply with the section cited above in that LPAs observed infants sleeping in 2 room with door shut which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Facility opened both bedroom doors during this visit.

Based o (record review)], the licensee did not comply with the section cited above in that S1 is issing TDAP and MMR immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of immunizations for S1 to the department by POC due date.
