LPA observed cabinets in all the preschool classrooms containing "snacks" for children, along with their cleaning agents in the same cabinets. The director was informed of the importance of keeping these items separate, and also which items to keep behind a latch, and which should be behind a lock. This poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed the playhouse the children were playing in and noted mud, dirt and debris. A staff was attempting to prevent the children from playing with the mud/dirt/debris. The other playhouse was full of dirt and debris. This poses/posed a potential health, safety or personal rights risk to persons in care.
LPA observed CLOROX powdered bleach in the "laundry room" as well as the "staff restroom", which is located in the hallway of the facility. Additionally, LPA also found powdered laundry detergent, and CLORALEN Aromas, another cleaning agent. These items are not behind a lock. This is being downgraded to a Type B citation due to LPA not observing any children in the hallway during todays inspection. This poses/posed a potential health, safety or personal rights risk to persons in care.

One hole was about 2.5-3 inches in diameter. 1 of 3 of the restrooms had a toilet that had a "V"-shaped crack at the top of the tank. Lastly, the ceiling tiles are not on properly, exposing wires/dirt &debris from the ceiling. This is being downgraded to a Type B due to the above not loosely dangling from the ceiling. This poses/posed a potential health, safety or personal rights risk to persons in care.
