Based on observation and record review, the licensee did not comply with the section cited above. Licensing was never informed of the addition to the home on the left side of "Living room#2". This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit an updated facility sketch as well as proof of permits for the room built to the left side of "living room#2", no later than the POC due date.
Based on record review, the licensee did not comply with the section cited above. 3 out of 3 staff did not have their Mandated Reporter Training (MRT) on file. Licensee and 2 present assistants did not have their MRT on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit an updated MRT certificate for herself, and her 2 assistants for "Child Care Providers". This is due to the department no later than the POC due date.
Based on (interview) and (record review)], the licensee did not comply with the section cited above in that safe sleep log not being used for C1 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee provided sample safe sleep form to implement today and will submit safe log for C1 by POC due date.
Based on [(observation), the licensee did not comply with the section cited above in that LPAs observed discharged fire extinguisher during this visit which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Facility will submit invoice/and picture of new or serviced fire extinguisher to the department by POC due date.
Based on [(observation) the licensee did not comply with the section cited above in that LPAs observed air freshener in bathroom, alcohol in cabinet, medicine/ointments, knife on counter and batteries during the tour which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee made items inaccessible during this visit. Cleared during visit.
Based on file review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review it was discovered C4 did not have sleeping logs available for review. Safe Sleep regulations were provided during inspection.
Resolution: Licensee agrees to submit sleeping logs for C4 to the Department on 8-4-2023. Sleeping log dates must include 7-31-2023 thru 8-3-2023. Licensee understands sleeping logs must be completed daily every time a child under 24 months is sleeping. Licensee agrees to read the above allegation and submit a statement of understanding to the Department on or by 8-4-2023.
Based on file review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review it was discovered S5 did not have a current Mandated Reporter certificate. S5 stated she did not take the training due to being on away from the facility.
Resolution: Licensee agrees to have S5 complete the Mandated Reporter certificate and submit the certificate of completion to the Department on or by 8-4-2023.
Based on the file review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review it was discovered S1 did not have immunizations available for review and S4 did not have proof of TB clearance available for review.
Resolution: Licensee agrees to obtain proof of required immunizations and required clearances. Licensee agrees to submit the proof on or by 8-11-2023.
Based on the file review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review it was discovered C1 did not have proof of current immunizations and C3, C7 and C8 did not have immunizations available for review.
Resolution: Licensee agrees to obtain proof of required immunizations. Licensee agrees to submit the proof on or by 8-11-2023. Licensee understands current proof of immunizations must be kept on file and available for review.
Based on the file review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review it was discovered C2 did not have a file available for review.
Resolution: Licensee agrees to obtain all required documents per Title 22 regulations and create a file for review. Licensee agrees to submit all documents to the Department on or by 8-11-2023.
Based on the file review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the file review it was discovered the facility did not have a roster available fore review.
Resolution: Licensee agrees to submit a completed roster to the Department on or by 8-4-2023. Licensee understands that a roster must be made available during inspection. Roster was provided during inspection.
Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. Upon arrival LPA was greeted by S1 who was providing Supervision and Care to nine children. S1 stated both Licensees were away from the facility. S1 also sated she has been working at the facility since November 2022.
Resolution: Licensee agrees to read the regulation cited above and write a statement of understanding and submit to the Deparment by close of business on 7-31-2023. Liceensee understands if there are more than eight children present there must be two staff at all times.
Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During the file review it was discovered S1, S2 and S4 did not have a Criminal Record Clearance and were not associated to the facility. S1 has been an assistant at the facility since November 2022. S2 and S4 are both Assistants & residents over the age of 18.Livescan for S1, S2 and S4 was completed 5-9-2023. Civil Penalty of $1500 assesed.
Resolution: Licensee agrees to read the regulation cited above and write a statement of understanding and submit to the Deparment by close of business on 7-31-2023. Liceensee understands S1 cannot return to the facility as her assistant until a Criminal Record Clearance is obtained and S1 is associated to the facility. Licensee agrees to provide and LIC 9224 to Authorized Representatives according to Title 22 regulations.
