Based on record review and licensees own admission, the licensee did not comply with the section cited above. Licensee was unable to provide an updated Mandated Reporter Training (MRT) during today's inspection. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of Mandated Reporter Training (MRT) for herself no later than the POC due date. This training is free, and can be found online at: mandatedreporterca.com The training due must be in "Child Care Providers".

Based on record review, the licensee did not comply with the section cited above. Licensee did not have LIC282 on file for 2 out of 4 children. C3 and C4 did not have LIC282's on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of LIC282 for C3 and C4 no later than POC due date.

Although LPA observed 2 cribs in the home, one was full of toys and the other one had diapers, multiple plush toys, a changing mat with blankets, and a small baby basket in it. Licensee stated that the 3 infants in care on this date, sleep on the couch when they do fall asleep. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Immediately, licensee will provide a safe sleep environment for the infants in care and submit a picture of this to licensing no later than the POC due date.

Based on observation and licensees own admission, licensee sleeps infants on the couch to sleep. LPA observed 3 infants in care during todays inspection. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA printed out the infant safe sleep regulations. Licensee agrees to read through it and send in a written understanding of what a safe sleep environment looks like. This is due to the department no later than the POC due date.

Based on observation, the stairs were not barricaded. Although licensee has attempted to put a gate up with bungee cords on it, the other side of it does not hug onto the wall, making the stairs accessible to the children in care. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of a secured gate on the bottom of the stairs, no later than the POC due date.

Based on record review, the licensee did not comply with the section cited above. 4 out of 4 children's files did not have an LIC282 on file. There were 2 LIC282's for other children (not reviewed on this date) that were mixed in with the files that were reviewed on this date. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of LIC282 for C1, C2, C3 and C4, no later than POC due date.

Based on record review, the licensee did not comply with the section cited above. 2 out of 2 infants under 12 months of age did not have an LIC9227 on file, during review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of LIC9227 for any infants that are under 12 months of age to the department, no later than the POC due date.

Based on record review, the licensee did not comply with the section cited above. LPA observed "Sleep Charts" hanging on the wall. The last sleep check was dated: 08-30-2024. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Immediately, licensee agrees to begin 15 minute checks for any infants under 24 months of age. Proof of these logs are due by POC due date.

Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. S1 Mandated Reporter Training expired on 05/17/2021 and S2 does not have proof of completion of Mandated Reporter Training.

Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. In reviewing staff files, LPA did not observe the following immunization records for S1 and S2: MMR, Pertussis and Influenza.

Resolution: Licensee agrees to obtain immunization and or provide a copy of the immunization records for S1 and S2 to the department via email by October 03, 2023.

Based on record review, the licensee did not comply with the section cited above. LPA observed the children roster incomplete and missing required information. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete children's roster by including the required information and submit a copy to the department via email by October 03, 2023.

Based on record review, the licensee did not comply with the section cited above. In reviewing children's files, LPA did not observe form LIC 282 in C1, C3, C4, C5, and C6 record. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete form LIC 282 and have the parent of C1, C3, C4, C5, and C6 of sign and date form LIC 282. Licensee agrees to submit a copy of completed LIC 282 to the department vai email by October 03, 2023.

Based on record review, the licensee did not comply with the section cited above. Licensee does not have a complete infant sleep log for C3 and C6 that documented the 15 minute sleep check. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create a sleep log with the required information, document the 15 minute sleep check, and submit a copy to the department via email by October 03, 2023.

Based on LPA observation, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. LPA observed prescription and non prescription medication accessible to children in care located in the interior door of the refrigerator. The oldest child in care at the time of the visit is 8 years old.

Resolution: Licensee immediately made prescription and non prescription medications inaccessible to children in care by removing all medications from the refrigerator. The Licensee placed all medications in a locked closet that is inaccessible to children. Licensee agrees to continue to keep all medications locked and inaccessible to children in care.
