In the on-limits restroom, LPA observed hazards in the shower, 2 under sink cabinets and the 3 drawers on the vanity. These areas had multiple hair and body products, as well as trimmers and shavers accessible to children in care. This is being downgraded to a Type B citation due to there only being 1 child in care. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During the inspection, licensee was able to immediately place these items inaccessible.

The on-limits restroom did not have poisons behind a lock. An accessible can of paint was observed between the on limits kitchen and dining area. Lastly, the laundry room contataining poisons was not kept behind a lock. However, the area had a gate preventing access. This is being downgraded to a Type B due to there only being 1 child in care. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During the inspection, licensee tied a zip tie around the latch to prevent the daycare child access for the time being. Although this provided a temporary solution for the child in care, licensee is asked to provide proof of a more permanent solution to keep items behind a lock. This is due to the department no later than the POC due date.

Based on record review, the licensee did not comply with the section cited above. Although licensee was able to provide proof of Mandated Reporter Training (MRT) for "general" training, there was no proof of MRT for "Child Care Providers". This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to gain access to the training online at madatedreporterca.com and submit proof of certificate for "Child Care Providers", no later than POC due date.

Based on record review, the licensee did not comply with the section cited above in 1 out of 3 persons. 1 out of 3 files reviewed did not have LIC282 on file. C2 did not have an LIC282 on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of LIC282 for C2, no later than POC due date.

Based on LPA record review, the licensee did not comply with the section cited above, documentation of the last fire and disaster drill documented was 12/16/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct and document a fire and disaster drill for October 2023 on the fire and disaster log including date and time. Licensee agrees to provide proof of fire and disaster drill documentation to LPA by COB 10/13/2023 via email. Licensee agrees to conduct the fire and disaster drill and document the fire and disaster drill once every 6 months.

Based on LPA observation, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. The fully charged fire extinguisher located on the kitchen counter is the incorrect size (1-A:10-B:C).

Resolution: Licensee agrees to obtain and maintain the correct size fire extinguisher (2-A:10-B:C) and provide a photo of the fire extinguisher dispaying the correct size (2-A:10-B:C) to LPA by COB 10/13/2023 via email.

Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. S1 and S2 does not have proof of completion of Mandated Reporter Training (AB1207).

Resolution: Licensee agrees to complete and provide proof of completion of the Mandated Reporter Training (AB1207) for S1 and S2 to LPA by COB 10/20/2023 via email. www.mandatedreporterca.com

Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. S1 and S2 First Aid and CPR expired 06/2022.

Resolution: Licensee agrees to complete and provide proof of completion of First Aid and CPR for S1 and S2 to LPA by COB 10/20/2023 via email.

Based on LPA record review, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. LPA did not observe the following immunization records for S1 and S2: Influenza or a statement declining the Influenza vaccine.

Resolution: Licensee agrees to obtain provide proof of Influenza vaccination or provide a statement declining Influenza vaccine S1 and S2 to LPA by COB 10/20/2023 via email.
