Based on review of records, licensee did not comply with the above statement. 1 out of 4 children's files did not have proof of Immunizations on file. C1 did not have immunizations for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of immunizations for C1 and submit to licensing no later than POC due date.

Based on interview and record review, the licensee did not comply with the section cited above as Licensee did not have documentation of any fire/disaster drill that was conducted within the last 6 months which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct and document a fire/disaster drill by the Plan of Correction (POC) due date. Licensee also agrees to send proof of the POC to Community Care Licensing (CCL) by the end of the business day on the POC due date of 07/27/2023.

Based on interview and record review, the licensee did not comply with the section cited above as C1 and C2 were both missing proof of the 15 minute sleep logs which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document the 15 minute sleep checks for C1 and C2 by the Plan of Correction (POC) due date. Licensee also agrees to send proof of the POC to Community Care Licensing (CCL) by the end of the business day on the POC due date of 07/27/2023.

Based on record review, the licensee did not comply with the section cited above as both the Licensee and the Licensee's assistant were missing proof of the Mandated Reporter Child Care Providers (AB1207) Training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the Mandated Reporter Child Care Providers (AB1207) Training by the Plan of Correction (POC) due date.
