Based on record review, the licensee did not comply with the section cited above. Mandated reporter training (MRT) for licensee expired on: 01-12-2025. MRT must be renewed every 2 years. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of Mandated Reporter Training for "Child Care Providers" and submit to licensing no later than POC due date. This training can be found online at mandatedreporterca.com

Based on observation, the licensee did not comply with the section cited above. There was a BB gun and alcohol stored in the bottom shelf of a closet in the hallway leading to the backyard. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee was able to remove the items right away. The BB gun was put in an off-limit area of the home, inaccessible to the children in care. The open alcohol was put in a cabinet and locked, also placing the alcohol inaccessible and away from daycare children.

Based on observation, the licensee did not comply with the section cited above. LPA observed an unfitted sheet on the crib mattress. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to get a sheet which is fitted for the crib that the infants sleep in and submit proof to licensing no later than the POC due date.

Based on record review and licensee's own admission, the licensee did not comply with the section cited above. The licensee shared with LPA that she is part of a group where providers share information with one another. A piece of information shared was that 15-minute checks were no longer required. LPA let licensee know this was not true. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to keep a 15-minute sleep log starting 04/06/2023 and submit proof of implementing it to licensing no later than the POC due date.
