Based on observation, the licensee did not comply with the section cited above as LPA observed a child in a baby walker upon entry into the facility which poses an immediate safety risk to children in care.
Resolution: Licensee immediately removed C2 from the baby walker and placed the baby walker in an off-limits area during the visit.
Based on record review, the licensee did not comply with the section cited above. In reviewing children files LPA did not observe immunization records for Child #1, 4, 5, and 8. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to request/obtain the required records to complete the children files. Also, copies shall be submitted to the Riverside Child Care Regional Office by 7/27/2023.
