Based on record review, 1 out of 3 staff did not have their TB test results. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. 1 out of 4 children did not have proof of Immunizations or enrollment in a school on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the facility did not comply with the section cited above. Room1 under the sink, LPA found a spray bottle of rubbing alcohol. In Room 2, LPA found a screw driver in a drawer, accessible to children. Additionally, LPA found 2 gallons of Bleach, "Real Kill Ant &Roach", multipurpose cleaner and other bottles with cleaning solutions in them. This is being downgraded to a "Type B" citation due to most of these items being behind a latch. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above two staff are not associated to the facility which poses a potential health, safety risk to persons in care.

(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).

Based on staffs own admission, the licensee did not comply with the section cited above. The director stated she has been here since August 2024, but did not notify the department or submit her directors packet. She did send it to a previous analyst, who no longer works for the department. The new director hoped licensing would make contact. This poses/posed a potential health, safety or personal rights risk to persons in care.

Per staff acknowledgment and admission of the allegation, the facility failed to provide a safe learning environment by raised their voices "yelling" making children feel worried/anxious while in care. "This is an immediate Health and Safety risk for the children in care".

LPA observed C1, C2 and C3 hitting each other and other children in care. Facility failed to rearrange staffing to ensure sufficient teacher/teacher aide(s) were present to meet the needs of the children in the class to protect the safety of children in care "This is an immediate Health and Safety risk for the children".

Per staff acknowledgment and admission of the allegation, the facility failed to provide a clean, safe, and sanitary environment by not changing and disinfecting the changing table and washing their hands between diapering children "This is an immediate Health and Safety risk for the children in care".

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 persons. 2 staff files did not have an updated Mandated Reporter Training (MRT) for "Child Care Providers". This poses/posed a potential health, safety or personal rights risk to persons in care.
