Based on record review request and licensees own admission, S1 did not have Mandated Reporter Training (MRT) on file for either "general" or "child care providers", but was observed supervising the children during todays inspection. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a MRT certificate for S1, specifically the "general" and "Child Care Providers" training's. This is due to the department no later than the POC due date. This training may be found online at mandatedreporterca.com Just a reminder, that this is a free-of-charge training.
Based on record review request and licensees own admission, S1 was observed assisting with the children; however, does not have immunizations on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of immunizations for S1 no later than the POC due date. Immunizations due include: measles (MMR), pertussis (Tdap) as well as Tuberculosis (TB).
Based on record review, the licensee did not comply with the section cited above. Licensee certificate expired 08/25/2020 and Licensee's assistant on 12/26/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and licensee's assistant agreed to provide Mandated Reporter Training Certificate by deadline to CCL.
Based on record review, the licensee did not comply with the section cited above. During records review the following children have incomplete Immunizations records Child N.2 to Child N.6 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to update children's records by deadline and provide copy of updated PM286 ( 6/95) by deadline.
Based on observation, the licensee did not comply with the section cited above. During LPA's physical inspection at the facility fire extinguisher dial was on red zone and in need to be recharge which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to recharged fire extinguisher or to purchase an new one and to provided proof of purchase ( receipt) by deadline.
