Based on record review, the licensee did not comply with the section cited above. Licensees Mandated Reporter Training (MRT) expired 08-12-25, putting her over the 2 year mark. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of Mandated Reporter Training (MRT) Certificate for "Child Care Providers" to the department, no later than the POC due date.

Based on [(observation), the licensee did not comply with the section cited above in that LPAs observed lighters, electric fly swatter, sharps, detergents accessible in on limit kitchen and bathroom cabinets and drawers which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee made all hazardous items listed above inaccessible to children. Cleared during this visit.

Based on [(observation) , the licensee did not comply with the section cited above in that LPAs observed three infants napping in play yards with blankets and additional blankets tied to outside of playards which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed blankets from inside and outside of playards during visit. Cleared during this visit.

Based on (record review)], the licensee did not comply with the section cited above in that 4 out of 5 chidlren's files had missing Parents Rights form LIC995 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee had parents sign 995 during this visit.

Based on [(observation) (, the licensee did not comply with the section cited above in that LPAs observed 3 infants, 3 preschool and 1 school age in attendance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During visit 2 children were picked up, putting licensee back into compliance. LPAs reviewed and discussed Health and Safety Code Section 1597.44 with Licensee.

Based on [(observation), the licensee did not comply with the section cited above in that LPAs observed banned Bumbo chair with no straps on site, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee threw Bumbo chair away during visit.

Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review LIC995A for C3. Licensee stated LIC995A is not available for review.

Resolution: Licensee agrees to obtain the LIC995A for C3 and submit to the department on or by the POC date of 08/18/2023. Licensee understands that all children must have all required documents on file and available for review during an inspection.
